STYSA PROTOCOLS EFFECTIVE AS OF JULY 31, 2020

  1. Any individual with a suspected case of COVID-19 should notify their respective team coach immediately.
  2. Any team with a suspected case of COVID-19 should notify their club safety coordinator immediately.
  3. Any club notified of a suspected case of COVID-19 in a player, staff member, coach, or referee should follow their COIVD-19 Action Plan and contact anyone within the club who may have had contact with that individual in the 14 days prior to the diagnosis of COVID-19. Please follow confidentiality requirements.
  4. Any referee with a suspected case of COVID-19 should notify the league director and referee assignor involved in competitions worked by that individual within the last 14 days.
  5. Any club notified of a CONFIRMED case of COVID-19 in a player, staff member, coach, or referee should notify any other organizations/teams involved in competitions with that individual in the 14 days prior to the diagnosis of COVID-19.
  6. Any club notified of a CONFIRMED case of COVID-19 in a player, staff member, coach or referee should notify the league director and the South Texas state office immediately.
  7. Any team with more than 3 CONFIRMED positive cases of COVID-19 is considered to have an outbreak within their team and should pause team activity until all participants have completed a self-quarantine according to CDC guidelines.
  8. All reporting should maintain confidentiality in accordance with the American with Disabilities Act (ADA) and other applicable laws and regulations.

IF YOUR TEAM HAS A SUSPECTED CASE OF COVID-19 YOU MUST FOLLOW THE PROTOCOLS OUTLINED ABOVE AND CONTACT YOUR CLUB COACH & CLUB SAFETY OFFICER IMMEDIATELY. 

 

IF YOUR TEAM HAS A CONFIRMED POSITIVE CASE OF COVID-19 YOU MUST FOLLOW THE PROTOCOLS OUTLINED ABOVE, AND YOUR CLUB SAFETY OFFICER SHOULD FILL OUT THE WDDOA COVID-19 REPORTING FORM AND THE STYSA COVID-19 REPORTING FORM IMMEDIATELY.  IF YOU ARE UNSURE WHO YOUR CLUB SAFETY OFFICER IS PLEASE GO AHEAD AND FILL OUT THE WDDOA COVID-19 REPORTING FORM USING THE LINK LOCATED BELOW. 

 

 

WDDOA GAME DAY HEALTH CHECK FORM

The WDDOA GameDay Health Self-Assessment is now available and can be downloaded, filled out for each game, and kept on file with the team manager for a minimum of thirty (30) days, unless, the team/club has adopted another verification process. 

We encourage Health Assessments to be completed before every game for players (completed by their parents), and all coaching staff.

To help you in this task, we have created a pdf form (available in two options) If you are going to use these forms, please choose the option you are most comfortable with. As a reminder, these forms were created as tools and are not required to be used. 

**Option 1: Should you not feel comfortable listing player names...this option ONLY asks for Player ID #'s OR Jersey #'s for identification of players. (Player ID numbers can be found on your Official WDDOA Roster)

Good Idea To Consider...

It is recommended team managers create a COVID kit with the following suggested items to bring to the field for every game (it is recommended you ask for donations from parents in order to keep your team’s kit stocked throughout the season, your club will not provide this):

Some Suggested Items to Put In Your Kit are: 
► Hand Sanitizer
► Sanitizing Wipes (for equipment) 
► Gloves (nitrile or latex)
► Thermometer (in case anyone did not remember to do a temp check at home)
► Trash bag
►Extra disposable masks
